It will be practically posted like it was wallpaper over the forum but I don't think Zeroroom is completely finished with the templates yet, which is why that hasn't been done yet.
The butt plate thing is very common. It's a result of using snaps instead och brackets and a maybe slightly too tightly strapped ab-belt. It's nothing to worry about.

I'm guessing about 4 cm wide, but that's just me guessing. Someone more knowledgeable in these finer details might provide a better answer. But I'm thinking these brackets were hand made and not to a specific width, so I think they varied in both width and height. But if you do it today to replicate you might want a uniform size of the brackets.

It's an easy approval and shouldn't have much trouble getting you centurion either. I have some suggestions to make it even better.
Your knee plate ends in a taper now, redo that and cut the ends so they stop vertically instead of taper.
Shorten the detonator clips and re-work the angles so that the keypad on the detonator is more horizontal and less of the clips are show when attached to the belt.
Rotate the right thigh a bit so it's more centered. Maybe it's like that normally, but on your pictures it's a bit rotated.
Most people do 20 on legs, 25 on back of the shins, and 15 on the arms. You can move away from these numbers if you need to, but keep the proportions for the right look. Yes, the thumbprint is on the left.
While it's still a little soft, massage it a bit by squeezing it between your fingers while it it's hole to get some of the gasses inside out of it. It makes for a smoother surface when you sand it.
Yeah, I know of those pictures. I've just never seen any scuffed paint on ears that has revealed any primer, but I guess it falls off together with the white paint whenever scuffed. I might have had a brain fart in my last post.

The bottom tool is what I associate with a hole punch, but the top one is one too, but of the fixed hole size kind that you use together with a hammer.
ABS paste could do it if your're careful. You may need to sand it smooth and polish afterwards to get a uniform surface though. Any kind of filler will work, but would also require paint on top.
If you look at the CRL on the 501st.com website you can see additional bullets in blue and red. The two optional FISD-levels are represented by those. Meaning the 2nd level award includes black+blue, and the 3rd level includes black+blue+red requirements.
