mold making

I am new to all of this I have watched videos of people vaccum forming stuff but how do they make the mold and make it screen accurate and what not. is there some kind of blueprint to follow in order to carve a mold cause I am all thumbs to this I tried to do it with clay but it looked like mashed potatoes when i was done lol!! any advise?
